Position Overview:
We are seeking a compassionate and motivated Physical Therapist (PT) to join our interdisciplinary team in our long term care (LTC) facility. The Physical Therapist will evaluate and treat residents to improve mobility, strength, balance, and overall functional independence while promoting the highest practicable level of well-being in accordance with state and federal regulations.
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
- Implement therapeutic interventions to improve strength, mobility, balance, endurance, and pain management.
- Establish measurable short- and long-term goals in collaboration with residents and the care team.
- Document evaluations, progress notes, and discharge summaries in compliance with regulatory and facility standards.
- Participate in care plan meetings and communicate resident progress to interdisciplinary team members.
- Provide staff education on safe transfers, mobility techniques, and fall prevention.
- Ensure compliance with Medicare, Medicaid, and insurance guidelines as applicable.
- Maintain a safe and clean therapy environment.
Qualifications:
- Graduate of an accredited Physical Therapy program.
- Current state licensure as a Physical Therapist.
- CPR certification.
- Prior experience in long-term care or skilled nursing preferred.
- Strong clinical assessment and documentation skills.
- Excellent communication and teamwork abilities.
- Knowledge of PDPM and skilled rehabilitation guidelines preferred.
