Job Title: MDS Nurse (RN).

Job Summary:
We are seeking a detail-oriented and experienced MDS Nurse to join our long-term care team. The MDS Nurse is responsible for the accurate and timely completion of the Minimum Data Set (MDS) assessments in accordance with federal and state regulations. This role is essential to ensuring regulatory compliance, optimal reimbursement, and the delivery of high-quality resident care.
Responsibilities:
- Complete and coordinate MDS assessments, CAA processes, and care plans in accordance with CMS and state guidelines.
- Ensure accuracy and timeliness of all MDS submissions and updates.
- Collaborate with the interdisciplinary team to develop and maintain individualized resident care plans.
- Monitor and track assessment schedules, including OBRA and PPS assessments.
- Ensure compliance with Medicare, Medicaid, and facility policies and procedures.
- Participate in care plan meetings, audits, and surveys as needed.
- Review clinical documentation to support accurate coding and reimbursement.
- Provide education and guidance to nursing staff regarding documentation requirements.
- Maintain resident confidentiality and comply with HIPAA regulations.
Qualifications:
- Current RN license in good standing.
- Minimum of 1-2 years of MDS experience in a long-term care setting preferred.
- Working knowledge of CMS regulations, RAI process, and reimbursement systems.
- Strong organizational, analytical, and communication skills.
- Proficient in electronic medical record systems.
Physical Requirements:
- Ability to sit, stand, and walk for extended periods.
- Ability to lift up to 25 pounds as needed.
