Position Summary:
The Licensed Practical Nurse (LPN) is responsible for providing direct nursing care to residents under the supervision of the Registered Nurse and in accordance with physician orders, facility policies, and New York State regulations. The LPN plays a vital role in ensuring the health, safety, and well-being of our residents while promoting dignity and quality of life.
Responsibilities:
- Administer medications and treatments as prescribed.
- Assess residents and promptly report changes in condition to the RN or physician.
- Document care accurately and timely in the electronic medical record.
- Monitor residents for changes in physical, emotional, and mental status.
- Supervise and support Certified Nursing Assistants (CNAs).
- Ensure compliance with infection prevention and control practices.
- Communicate effectively with residents, families, physicians, and the interdisciplinary team.
- Participate in care planning and resident evaluations.
- Respond appropriately to emergencies and resident needs.
- Maintain compliance with all federal, state, and facility policies and procedures.
Qualifications:
- Current New York State Licensed Practical Nurse (LPN) license in good standing.
- CPR certification preferred.
- Long-term care or skilled nursing experience preferred, but new graduates are encouraged to apply.
- Excellent communication and organizational skills.
- Ability to work effectively in a team-oriented environment.

Schedule:
- Full-Time and Part-Time positions available.
- Day, Evening, and Night shifts.
- Every other weekend and rotating holidays required.
